Supervision Waiver

The City Smarts New Client Agreement provides that a parent or legal guardian will be present in the home for the duration of any session conducted at the student's residence.

By signing below, I waive that requirement and consent to my child working with a City Smarts employee at my home without a parent or legal guardian present.
