Supervision Waiver

The City Smarts New Client Agreement requires a parent or guardian be present while the student works with a City Smarts employee in their home.

By signing this consent form, you waive condition one and acknowledge and give consent to allow your child to work with any City Smarts employee your my home without a parent or guardian present.

This agreement remains in effect until written agreement of both parties.
